Program Coordinator/Educator- RN/RD Preferred (Full-time)
The Program Coordinator/ Educator is responsible for the direction, coordination, implementation, and evaluation of the Defeating the Deadly Double (Diabetes and Depression) Program. This is a 2-year grant funded program (February 2018 – January 2020) designed to establish an effective continuum of care for health center patients diagnosed with both diabetes and depression.
REQUIRED KNOWLEDGE, SKILLS, COMPETENCIES:
- In-depth knowledge about current diabetes treatment management.
- Familiarity with depression assessment and therapeutic approaches, via direct service or integrated practice and/or referral relationships.
- Ability to lead and effectively manage groups.
- Ability to develop a collaborative, therapeutic alliance with individuals.
- Competence in program management and educational program design and evaluation.
- Basic understanding of grant-supported programs and reporting requirements and methods.
- Basic computer and software skills, including use of Internet and e-mail, database software, electronic health records.
- A healthcare professional with a passion for diabetes/chronic disease care
- Preference for RN or RD, with CDE or CDE-eligible
- Minimum 6 months’ experience providing diabetes education, and at least 80 contact hours within past year in diabetes and diabetes clinical and educational subjects
- Bilingual in Spanish a plus

Under the direction of the Chief Medical Officer, the psychiatrist works closely with other team members including physician assistant, therapists, case managers, and medical assistants. New Horizons Healthcare’s Behavioral Health Services is part of an integrated interdisciplinary health center team, and requires the ability to co-manage care within the center, including medical, dental, pharmacy, and health education professionals. Clinical responsibilities include evaluation and assessment, medication management, crisis resolution, and consultation, as well as administrative responsibilities in electronic records management, and strategic planning for the department.
